Empire Blue Cross Blue Shield Doctor Note
I was told today that I need to have a note from my doctor saying that I have been on a diet for six months. Just wondering what other Doctors have written for this that have gotten insurance approval? Everything has been going great with all my doctors appointments and now I am concerned that this could hold me back. Thank you!
I'm not sure what all my physician wrote, but I brought in log books from Weigh****chers (on & off for YEARS), Jenny Craig receipts, and proof that I had once been on MediFast when living in West Virginia. Plus, we discussed my weight & what I'd been eating at every doctor's appointment, so I know there was a paper trail. Plus, my primary care physician SUGGESTED the surgery and gave me a referral.

I believe that each individual insurance plan has their own pre-requisites. I have BCBS POS plan so mine was fairly easy. I called them when I had my first consult and asked them what their requirements were, basically dietician twice, support, cardiac, pulmonologist and psychiatrist. I know that I have seen some ladies there that have to go through more rigorous things to get approved. I suggest that you call them and ask them what they expect so you can be prepared. On another note I have heard from those who had the six months dietary restrictions that they are glad their insurance made them do it.

Sorry to tell you this part of my story but the 6-month did hold me back!! I needed 6 consecutive notes of weight,a dvice, etc. from my primary, which he gave me. We felt my whole medical history was about weight loss but no, the insurance co. wanted that six month supervised diet with dr. Make an appt. so you have it. I just had to stop fighting and give them what they wanted.
