Help with appeal letter

Can anyone please help me format an appeal letter for anthem BC/BS? I was denied for lap band and need to appeal the denial but I need help formatting the letter. If anyone has a sample that they used, I would appreciate the help. Thanks LM175

LaShawn: If you look through the older CT posts you will see a message from Amy K. She was denied then approved for the band. I am not sure if she has the same or a similar policy to yours but it did say Anthem. The post was not that far back. Hope this helps. Sorry I can not be of more assistance. Best of luck to you. Jessica Thanks for your response. I am also wondering how come there isn't more support and guidance with writing the appeal letter. My doctor is Dr. Valin from St. Raphael's hospital, in New Haven. This reminds me of research papers for college. It's also frustrating. Hopefully, with guidance from old post with examples of appeal letters this process will be over soon. Pray for Me. What I don't understand is why YOU are writing it and not the Doctor? With Dr. Valin are you reponsible for getting your own approval? If so, then what exactly does the office do for you? I don't mean to sound rude but if this is how they handle things BEFORE your surgery, how is the aftercare??? Jessica

Hi LaShawn, You can go to the top and click on library and then type in appeal letter and it will give you old posts with info and some peoples profiles to check on for appeal letters. You will also get a lot of help asking this question on the main board. Good luck Heidi 267/170?/147
