Insurance Appeal Letter

Hi, Quite some time ago, a sample of an insurance appeal letter was posted either on the CT board or the main board. I am not sure which. I had saved it , just in case, and now I need it.Dr. Aranow's office submitted my chart today to Anthem, and I do have coverage, but only for RNY, and not the Lap Band that I am planning on having.She said I should be prepared to be denied, but to have my appeal letter ready to go. The bottom line is that I have no idea where I saved it,and need your help if you have a sample of an appeal letter, that I can use as a reference.Thanks for everyone's help. Marcia

If you go to the "library" and type in appeal letter, a sample of one shows up. Not sure if it's the one you were looking for. Anne
