hi Lori I just recieved a change of address post card; I'm sure you will be getting one soon....just in case 400 Saybrook Road suite 110 Middletown, Ct 06457 I'ts a brand new building right before the outpatient center.It's on the left high on a hill.....WITH TONS OF PARKING angel ct.
