The state should be able to provide you with a list of psych's.  It doesn't have to be a psychiatrist....can be a therapist, etc.  I had the same prob as you - I have Anthem insurance.  I had to get a provider manual and spend hours calling people leaving messages to see if they did WLS evaluations....I finally found one in Enfield - McIntyre Consulting and they were awesome.  Good luck!  It's a lot of foot work but well worth it.

I was told right from the start that insurance would not cover a psych evaluation for the surgery--I had to pay $300 out of pocket...best investiment I have ever made!  because it allowed me to get the surgery...the evaluation itself was pretty stupid!  they asked all these questions--like how did I feel when my mother died...I made the mistake of saying "sad" she then asked me another 50 or so questions on my "depression" over my mothers death...it was pretty dumb...when I got the evaulation, it said I had situational depression....who is not sad when their mommy dies???  anyway, I passed... good luck Jennifer

I went out of my way to go here as I was told she would work it out with my insurance, Healthnet. However she billed it, I just paid my regular co-pay of $10.
