I was wondering if the form that we have to fill out with all our consultation appointments should be faxed after we have all the appointments scheduled or after we have all the forms completed. Your help would be greatly appreciated. Thank you, Delicia

Hey Delicia, I would wait till the appts are completed. Reason being the cardiologist may schedule a stress test and you will not know that till you see him. Same with pulmanary. The office will NOT take the form unless all appts are completed. GOOD LUCK!!! Carrie

I guess I'm the odd one out. I scheduled all my appointments and then faxed it to them. I added a cover sheet and told them if they wanted it done a different way to please let me know. They never said anything. In fact, I was there today and they were fine with everything I had done so far!

Stop dancing Delicia. I was there for my 1st nutrition appt. I have already cleared my Psych...he thinks I'm normal....lol I have had Pulmonary and Cardiac, but have to go back for both. I have a Sleep Study next week. My second nutrition appt is next month. I only have to lose 2 pounds! Thank God for small favors! Judy and I discussed it, and I should most likely have surgery the end of February, start of March.

Still it sounds like we may be on the same time track when it comes to the surgery. I just have to do a sleep apnea test on the 14th of november. I have melissa as a nutritionist and not judy. I know I was surprised when they said I only had to lose 2 pounds. I guess it's when you meet with the surgeon that he tells you you have to lose more.

No, I think that's it. Judy told me when I come for the second appt, what ever weight I'm at, I have to sign a form that says I won't gain anymore. She says I can still lose IF I WANT, but it won't be a requirement. I just can't gain anymore

Thank you everyone for your response. I will hold on to the form. I have my 2nd nutritional appointment on 11/6 and my sleep apnea test on 11/14. Any ideas on how long it will take to meet with the surgeon or how much longer I have before I'm actually getting the surgery. (I'm just looking for a time frame or a good guess!) I'm just so impatient but I'm trying not to be. At least it's been 15 days smoke free for me but no cigarettes is making me irritable and a little

After I finished all my appointments, they called me about a 4-6 weeks later and made an appointment for me to see Barba about 6 weeks later. I finished all my appointments mid august. I will be meeting with Dr. Barba on 11/16. I was told at the time that surgery is normally scheduled about a month after that. I dont know how true that is but we shall see. Good Luck.
