How Do I contact Dr. Carlos Barba???

Does anyone know how I can contact Dr. Barba's office?  I had surgery two years ago and I wanted to make an appointment to see him due to some problems after my surgery.  I moved to NY right after my surgery and I'm unsure how I feel about my current surgeon and wanted Dr. Barba's opinion on my hernia and bowel obstruction.  Thanks to anyone who can give me information!
