DIET HISTORY: any examples out there of how to write it up?

First, congratulations on making this journey! You'll find the people on the CT boards to be a fantastic bunch and if ever you have any questions, this is the place to come!
I have a form I believe I downloaded from some Dr's website which I filled out and brought with me to my first visit with Dr Reinhold. They were impressed that I had captured pretty much all the information they were going to ask of me anyway!! I will email it to you - it includes a section on diet history, so that should help. And I went ALL the way back! As much as I can remember anyways. LOL!

I made my own diet chart on my pc for Dr. Aranow and he said it was very good and what is needed to submit to the ins. co. to get their approval.
It had 4 columns: Name of diet, Dates (i.e. 1-1-80 to 7-1-80), Lbs lost, How long weight stayed off.
The dates were approximate as best as I could remember and I included photocopies of whatever I had to help show that I have invested a lot of time and money in weight loss classes and products. I had cancelled checks from Weigh****chers, Lean Line, etc., as well as my charts from those classes that showed not only dates, but the weight loss as well. I also made copies of before and after pictures to show the fluctuation of weight over the years.
Like I said, Dr. A liked it, I just hope CT-care does too
