Samples of Letters

Goodmorning All... I was asked to give my pulmonary Dr. and Primary Care Physician a sample letter and at least let them know what there release for surgery is supposed to say. I thought I had come across a whole bunch of samples on ObesityHelp.com but it may have actually been in someones profile. Any Help will be appreciated. The smoother this process goes the better. ~Julie~ Pre-Op

Julie, I have a good sample of a letter I used for my PCP and you are welcome to use it. I will send it to your email address. It worked wonders for me and my PCP didn't change a thing like I thought he would he just signed the one I sent him!! Let me know how it works for you! Cheryl
