Does anyone else have this insurance in Colorado........
I noticed that a patient of Dr. Johnell had surgery last year and evidently we have the same insurance however, I do not have the name of the person.
The insurance is Colorado Contractors Trust/Sloans Lake and the RNY surgery was posted as being performed 3/14/04, I believe.
Any information would certainly help as I just sent in a long overdue appeal yesterday and am hoping for the best.
