Insurance reimbursements?

Hello,I was wondering if anyone has got their $600.00 assistant surgeon's fee back yet after HMO COLORADO has paid Dr. Snyder. From what I understand, all fees were covered with this type of insurance and if you had your surgery done by Dr. Snyder and Patrick Sawyer, you can get reimbursed for Patricks' fee. This fee was collected up front before the surgery. Any one who has info can help me on this. Thanks
