BMI of 34 - Is there any hope of lap band approval?

Are there any surgeons in the state of Colorado that may approve lap band surgery with a BMI of 34?  If there are, how about insurance coverage? I have Cigna and they told me that my BMI doesn't have to be in a certain range as long as the physician approves the procedure. However, most surgeons won't do it unless your BMI is >40 or >35 with risk factors. Again, my BMI is 34 with joint pain. What other risk factors are taken into consideration? Any information would be greatly appreciated.
