ID card?

I am looking for the ID card that you can print off to take into restaurants, etc. I'm quite sure I saw it on this website but cannot find it again. Can anyone help?

Click on 'My OH'. On the left side, in the first box, the last line says something like 'restaurant card'. Hope this helps!
