Who had/has Anthem BC/BS?

Hi everyone, With this insurance having a 7500 cap on Bariatric surgery, I am wondering how all of you managed to pay for your surgery. Did you have to have the money upfront or did you pay as the bills came? Any al all info would help alot!!!
