Dr George Mueller in San Diego - Program Fees?

I'm wondering if anyone who has used Dr Mueller in San Diego for either RNY or VSG can tell me how much that they ended up paying out of pocket, after insurance, for program fees and other non-covered expenses.  I have a $400 insurance deductible, and then everything should be covered 100% for the surgery, but I'd like to plan ahead if there are any other expenses that would not be covered by insurance (such as the nutritionist, follow up care, psych eval, etc.).

I'm currently in the process of them submitting for authorization for gastric bypass. The paperwork I got from them shows that they charge a separate fee of $300 for all the extra costs, ie. follow up, nutritionist etc. That is on top of your hospital co-pay for the surgery, any deductibles, or office visit co-pays etc. When I had my psych eval, it was considered a specialty consult, so I had to pay a co-pay, and the same with the medical clearance MD. It really depends on your insurance and how much they cover. The end amount is different for everyone I'm sure. But, the $300 program fee is due at the pre-op appt, upfront and in cash LOL, according to the pamphlet they gave me.
