Dr Eslami Questions

Does anyone know Dr Eslami and if he does the DS? My medical group has referred me to him and I can't find any info even hinting towards him doing the DS. I called the office and left a message for Lynette the coordinator but she has been out sick and i'm going crazy trying to figure this out. I had an Independent Medical Review decided in my favor saying that I can have a revision, but the group referred me to him instead of the surgeon that the request was for. Any info would be greatly appreciated. Thanks in advance!
