CA state disability (xpost)

 CA state disability is saying husband needs to turn in a medical certificate from Dr. Aceves in order to qualify for his disability payment for the time he was off.  We've contacted his office and received papers (like a diploma and other certifications), but no "medical certificate" that is required by the state.

Anybody know how to solve this problem?

The form you need is on the State Disability Website.  You can print and take it to the Dr to have him fill out his portion which is page 3 of the form set.  They should word it as  "Medical Certification" but they dont.  Here is the link   http://www.edd.ca.gov/Disability/DI_How_to_File_a_Claim.htm

You can fill this out online and print it will have all the information you put on it ready for the Dr to put his info on.

In addition to what Sue told you about, when you give the form to the doctor to complete, do not let them mail it in because 90% of the time it will get pushed aside and forgotten by the doctor.  Have him complete is during your appointment or make a scheduled time for you to pick it up so you can mail it yourself so you KNOW it went to the SDI offices.  THIS IS VERY VERY IMPORTANT, unless, of course, you don't care if you don't see a penny for several months :-)

Remember too that there is a 7 day unpaid waiting period for Ca State Disability, so if you haven't done so, check with his employer to see if they can integrate sick/vacation time during the waiting period and to make up the 1/3 difference in wages for the time that will be covered by the state.  I do benefits and leave of absences for a living, so hope this helps.
