For blue cross anthem ppo..dot your i's and cross your t's.  This letter helped me:

I filled it out with all my particulars, asked the doctor to sign it and faxed it to my surgeon's ofice, who sent it to BC.

Good luck,

Thanks so much...I need all the help I can get.  I will be going in at the minimum bmi.  Do you know how many co-morb's they require? Most is two..but when I called them..they really don't tell you!
Do you know where  I can find the other form mentioned in the letter?
Thanks so much
Kari

I'm not sure what BC requires.  Your doctor should know what gets approved and what doesn't.   I had sleep apnea, leg edema and type 2 diabetes.  I didin't need the other form, so I didn't look.  Google it, putting quotes around the form name.

Good luck,

Hi Kari ~

Kaiser out sourced me to UC Davis.  I like their program a lot better than Kaiser.

Annabelle,
I saw that you had dr. alli...I was going to post to you, but then I saw  you were talking about kaiser...so i thought it was a different dr.!
Well...now I can ask you the questions::))))
How was their staff, and were they willing to fight for you and your need for the surgery?
How did the process go?
Thanks
Kari

What I liked about UC Davis is they give you a surgery date before you lose weight.  I was given a date 6 months out and of course if you get everything completed early you can have that date changed and moved up.  That inspired me and helped me, and so I was able to get 43 lbs off.  Dr. Ali is very nice and personable.  The staff is kind also.  There was no fight for me at UC Davis.

Good Luck,

Hi,
We are practically neighbors, infact I used to live in Folsom (Willow Creek/Flower.  We work with the Dept. of Corrections, close by.

Moved up the hill to El DOrado Hils, then further up to El Dorado (Poor Reds).

Just saw where you were from and wanted to say "hi".

Hi Kari,
You just have to be patient!!  Which is not my virtue.  I had surgery a year ago and have lost 82 pounds.  I get frustrated when I compare myself to the others on the board when I see them lose 140 pounds in a year!!!  I am a juvenile diabetic, so there in lies my problem.

I don't get sick on anything.  I can eat anything and have only been sick once when I ate too fast.  Just follow your doctor's advice and listen to them.  No drinking right before and an hour after tou eat.  Eat your protein first, and remeber those vitamins!!  Exercise is my enemy, I hate it.  I knew if I were more active, I would be to my goal (20 more pounds).  But I am thrilled with my progress and feel wonderful!!!  It's so cool going in and buying cute clothes right off the rack.

If you have any questions, just ask.  We can exchange e-mails and phone numbers if you'd like; and, maybe we can meet for coffee (decaf) sometime in Folsom, as I am always there...
