I bill insurances for a living, let me tell you, they are absolutely, positively CRAZY!!  I am so sorry that you're having to deal with this!!  I swea they deny & make things difficult just so they won't have to pay the little bit they pay the doctors.  We most certainly need to re-vamp our whole insurance industry....but that's another issue. 

I think you should at least consult with an attorney.  I'm sure the doctor's office will fight for you!  They want to do surgery & get paid, almost as much as you want the surgery!!

Dammmitttt! I can't believe they've done this.  I'm going to email you a letter I had my doctor sign and send it to your doctor to send to the insurance company.  Change the exercise and medical parts that pertain to you.  They are just delaying things. 

If that doesn't do it, see Dr Jossart in SF  They have the approval process down pat and he's an amazing surgeon.  You can still attend support groups near your home.

whats the IL at the end?  I've never heard of it that way.
I have blue cross ppo which I guess is now anthem blue cross.  I am at the beginning...of wls journey, just getting my sleep study done.  Your 2nd denial is worrysome to me....

Oh, it's Blue Cross Blue Shield of Illinois, PPO.
Just try to give them exactly what they ask for...........that's really all I was told but I just don't get it, because I thought i did.  i don't know what more they want.

I have a copy of  my letter on my page. Feel free to copy and use it. I never received a formal denial but stalls from the PCP, the group and BS at every turn.  Only once I sent this letter CERTIFIED mail with the threat of bringing in an attorney did I get instant results!!! Try it!
