My Doctors Letter to the insurance company

Hello to everyone here.  I am new here just getting started learning the site. I am still trying to get all my requirements filled for the lap band surgery. I have talked to my general pratice doctor about this letter to the insurance company. He is all for me having the surgery and he will write me the letter. He is not sure what the insurance company is looking for in the letter.  My insurance company said i need a letter from my gp doctor with any comorbids. (Not sure on the spelling) Has anyone else had to do this letter? Could anyone give me some idea of what this should include/ Thanks Tony.

The letter needs to state all of the ways that your obesity affects your life and for how long you have been morbidly obese. My doc must have written a great letter because HealthNet approved my referral to the surgeon in under a week! When I asked her what she wrote, she said she just put down everything I told her! and a 5 year weight history ..... along with the length of time and type of treatments for all illnesses.... and the various ways you tried to loose weight
