WEll, I am 3 years out and had Health Net.  The question is HMO or PPO.  You need to go to their website and look for the policy.  It is there.  Their requirements are pretty much the same as everyone.  They do have a designated facility list the they use.  So that is what you will need to checkout. I have Healthnet HMO with Monarch as the provider - I was approved in less than one week.  I give a lot of credit to the staff at my surgeon's office. They helped prepare the history of my weight over the years & documented the high blood pressure, etc.  For me the hardest part was waiting, even though the approval came thru so quickly.

I had Health Net with my surgery almost 2 years ago. I had an HMO and my medical group required 6 months of weight management classes prior to a referral to the surgeon and then after that the process still took sometime for me because I had a few issues that had to be taken care of prior to surgery. You have to start with your PCP if you have an HMO and follow the process they have in place. Make sure that they document ALL of your health issues in your chart. Good luck Lisa
