PS consult Monday

Hi all, Well, Monday is my consult! I am super nervous..... Do I tell the doc all I am interested in having done? Do they weigh me clothed or in a gown? I am worried he is going to think I weigh to much for PS!! If I want to choose him and schedule, do I need to give a down payment on Monday? I am super nervous......but excited! Dh is going with me! :0)
