Out of Country form

Hi all
I'm wondering where I would find the MSP Out of Country Form? Can someone send me a link??
Also, I see that the form must be filled in by a BC specialist. Does it have to be a Bariatric Surgeon? I thought I read somewhere that a person had there Endocrinologist filled out their form. What other Specialists are excepted??

 :/  I guess there us no point in asking about the form.  Has anyone gone to Seattle for The Sleeve and paid for it out of pocket?  If I choose to do this, how much am I looking at paying?

If you get your GP or Specialist to refer you to West Coast Bariatric Surgery, in Richmond, it would be covered by BC Medical (MSP).  The doctors (Nguyen & Sampath) are both excellent bariatric surgeons.  Check out website www.westcoastbariatricsurgery.com   Wait time for first consult is about a year and then surgery date is given quite quickly, once you have completed their program, which can be started as soon as a referral is given, you do not have to wait until first consult.

 bamf, funny you should mention this!  I did that very thing yesterday :)  my concern was the long wait.   I have already ordered the book too.

I know of several people that finished all of the required program classes, then phoned the office to let them know that they were all done; they got their first consult within 2 weeks of them letting the office know of their status.  You must also keep a journal of everything that you eat/drink, plus all exercise/activities that you do each day, as well as the amount of weight that may come off while following their program, because you will need to bring these journals to every appointment, to prove that you are serious, and ready for surgery.  The doctors like to so the surgery within 3 months of first consult, as long as you have completed all of the test & specialist appointments within that time.  I had to wait just under 6 months, because I was out of town for 2, periods of 5 weeks, and then came down with pneumonia at the time I should have had surgery, so had to wait a bit longer.  I lost 54 pounds during the time that I was following the program, before surgery, and the weight came of very quickly for the first 6 months, but slowed down a bit after that.  I had Gastric Bypas (RNY) on Oct. 3/11 and have lost almost half of what I started out with.  I am getting fairly close to goal, but I have heard that the last 30 pounds are the slowest/hardest to lose.  GOOD LUCK on your journey.

I am inspired by your success.  At the stage I am at, I struggle with whether to proceed with this journey.  It's good to read of other peoples' journeys.

Now that my referal has been received by WestCoast Bariatric, are they going to contact me to let me know of these required classes BEFORE the first consult?

No, they probably will not.  You usually get everything at first consult.  They may send you a letter listing what you need to do, but you can self-refer to get a jump on it.  I got my letter after my first consult, but some of the program requirements that were in place when I started my journey, are a bit different, so I am not sure about the timing of the letter.  You can phone Garrat Wellness Centre @ 604-238-8004, to register (there is a cost of $90) or email the nutritionist directly @ [email protected], if you want any more information, and to find out when the next class starts (Winning at Losing).  You should also start reading "Weight Loss Surgery for Dummies".  Also, you could email the surgeons (email addresses on the website), if you have any specific questions regarding the types of surgeries, or about the preparations required.
