Since this is all new to me may I ask why are you going to USA when there are places in Vancouver and Victoria?
I live in the Rockies half way between Van and Calgary most of the time and Texas / Mexico the balance.
Since I am away from Canada right now... may I ask if BC Health Services will pay or co pay a US surgery?
Due to the fact that I travel alot I am leaning more towards the "sleeve" due to the maintenance factor with the belt.
Any comments would be appreciated.

Congrats on your sucessfull OOC application. I had my form filled out by a surgeon who was a bariatric surgeon but was not currently able to perform WLS do to lack of OR time, he is no longer filling out the forms now, I was the second to last person that I know of who's forms he did fill out. I have heard that you can have an endocronologist fill out you OOC form, I am so glad to actually see someone who has gone this route and was successful! initially i was advised via all the boards that only a specialist or bariatric surgeon could submit OOC forms not a GP.
I had my endocrinologist do them and was approved in under 2weeks to start the process with Virginia Mason. i do have many other co-morbidities and high BMI so maybe this helped. We submitted everything off the bat, all test results, newspaper articles of recent cutbacks and risks in waiting.
Why did i go to this route? I had started process here in Canada, made several trips back and forth to Victoria to see Dr R before he retired and was at the final stage with expected surgery date for the fall of 2009 when VIHA started cutbacks, Dr R's office was clear it would now bump me into late 2010, i wasnt prepared to just sit and wait and decided to try OOC or pay myself if i had to.
Puget Sound Surgery Center i started with, great people, seem to know what they are doing and if you pay well your looking at no more than 2-3 weeks wait. MSP will not pay for PSSC, they have a deal with VM for discounted rates.. so i went this route.
