What is this tapp form

I have heard people talking about a form that your doctor can help you with to help with your cost  for traveling, I have to go back to see Dr.Tang the end of the month, I have already seen him once last month. But I didn't know I could get any help with the cost.. my doctor tell me nothing!..
If anyone knows what I'm talking about and could help me find out where I get these, I would be very grateful..

Tap form is travel assistance program, phone Dr. Tang's office and ask the receptionist to send you a form, she will fill it out, then you fill out your part, and you have to call to get a confirmation number depending if you are flying or going by ferry,instructions are on the form once she mails you a copy of the TAP form.

Also if you are going to stay in a hotel there are ones that offer medical rates in Victoria

go to www.health.gov.c.ca/msp/mtapp.tap_patient.html 

will how you a listing of hotels you can call and also about the tap form.

Thank you so much for your help, i will do that on Tuesday, My doctor doesn't tell me things like this..
This will be my second trip over so if I can save a bit of money that will be awesome

My doctor didnt tell me either, I had heard of it on the VBS website.

Also keep your receipts, food, hotel etc because not sure but might be able to put them in as expenses for taxes  I kept a copy of the TAP form, phoocopy, so I could prove why I was in Victoria for my tax receipts if asked.

Wishing you a good trip over to the island.  I had my first visit with Dr. Rusnak two weeks ago, and will have to see a respirologist and dietician over there before my surgery.  Ive already seen the encrendonologist on the island,m but have to see one here as well on Tuesday, had my scope done over on the mainland, and had the overnight oximetry done on the mainland as well.

Hi Kelly, when you do phone into the TAP line, make sure you listen to the prompts. and respond accordingly,  I made a mistake when I was on my way to Victoria on Thursday and the code they gave you I had at the bottom, was wrong,  the code only allowed for one vehicle and a driver and I had to have an escort,  we had to pay for my escort because the number was wrong, and the lady we had at the ferries was not nice at all.  just wanted to shoo us away and no help at all.  Luckily I still had a return trip that I used otherwise we had to pay for the ferry as well.  I sat at the ferries and rectified the problem, only issue is, after you've paid, no refunds.  just a little tip from my side...
