I need help with filling OCC forms~

I have a few questions about the form, first part where it says date of departure, so would I need approval from the surgeon frist before I send in the forms? The one the section B part, it needs to be completed by how many doc/ and specialist. So I have an appt. date with doctor Tang, does fill out the form as well, and will he charge me a fee. When I have my appt with doctor Meneghetti, does he also fill out this form, and when I start having all my testing done, do they fill out the form as well.. I have my paper work from doctor Hunter, do I take these forms with me for him to fill out as well? Or do I just need on surgeon or specialist to fill out the form? It also wants to know the address of the hospital where I will be having it done..thats the thing, I am seeing a few doctors to try and speed up my wait time..so who ever I get in with first is where i will be going, But!! I need the ok from MSP and OFC first? does anyone understand what the heck i'm talking about...lol If you have a min and could help I much be a very happy girl and muchly appreciated... Hi Kelly i can explain a little to you as i've just finished the approval part. The OOC form has to be signed and filled out by a specialist. Dr. Meneghetti or? I don't know if Dr. Tang is filling out the forms or not, i know Dr. Amson isn't. If you read back a few pages you'll get lot's of information from the ladies that have already had their surgery. I do know that to have OOC approved you need to have a BMI of at least 50, they will do a little under if you have at least 2 co morbidity's like diabetes or sleep apnea. Dr. Meneghetti does charge $200 to fill out the form, and only accepts cash. The first form you mentioned is the one you submit after your surgery is over. You'll be accepted ONLY with a BMI of 50+ and/or 2 co morbs??? I'm a lightweight at 250 and my mbi is around 38/39 but I also have high blood pressure, cholestrol, and tachycardia..... I'd sure like to know before I shell out $200 to Dr. M~~~

Dear Kelly, Email me privately with your home number, contact info and I can help you with the form. It is a little detailed and complicated and you have to make sure to do it properly and you have to have the pre op tests tests done, etc before you mail the application in. I was the first Canadian to get MSP approval to go out of country to Seattle to Virginia Mason Hospital for WLS on June 26, 2007 with Dr. Hunter. I have helped several people with their approval after me. There is lots to go over. And there are critieria to qualify but even then not everyone gets approval as the other posts have mentionned.
