Quick Insurance question...

Does anybody have Pacificare? was it easy to get approved through them? I know that my policy covers the surgery but they did not tell me the criteria i needed to meet even after i asked. Do they require a 6 mo medically supervised diet? Also I was doing research on Mexican-American Obesity and found some statistics about current and future problems i could face.( My only co-morbidity is high Cholesteral.) It's kind of like "If you don't approve me now you'll be paying for it later" kind of deal. should i turn these in along with everything else. thanks!
