Which Phx employers exclude WLS?

Hi. I live in Tucson and am interested in knowing which major employers in Phoenix contractually exclude weight loss surgery from their insurance coverage. For instance, here in Tucson, Pima County and Tucson Unified School District employees are both excluded. Any info would be appreciated. Thanks. - Stephanie J
