I went to Lubbock Tx to meet Dr Frezza and had my initial Consult. I was denied out of network to AZ. So far I have a date of 25 October for Surgery. Dr Frezza's office submitted to my insurance for authorization and my insurance received the paper work on the 23 of September and it is still pending. Since Dr Frezza is a contracted dr, there should be no problem getting the approval though.
