A little late in the game for this to arrive to me, after already being
approved, but I got a letter from the AZ Dept. Of Insurance today.
Long short, it states "HEY, we can't help you because your husbands
employer, Brinks, is self funded" BUT, here is what you CAN do:
PROPER PROCEDURE, WHICH COULD POSSIBLY RESOLVE YOUR ISSUE, IS TO FOLLOW THE APPEALS PROCESS AS IT IS OUTLINE IN YOUR BENEFITS
BOOKLET. IF THE OUTCOME OF THE APPEALS PROCESS IS NOT TO YOUR
SATISFACTION, YOU MAY THEN TO CONTACT THE UNITED STATES DEPT. OF LABOR, 790 EAST COLORADO BLVD., SUITE 514, PASADENA, CAL 91101
Maybe this info can help someone else out there. Don't forget to get
your state politicians involved!
