phoenix health plan
I have Phx Health Plan along with (medicare) Abrazo Health. I went thru a rough time due to Dr. Simon backing out on his pts, so had to try and get a second referral for a Consult with a new doctor. Long story short - I wrote an Appeals Letter to PhxH. P./ Abrazo and got the denial over-turned!!!!!! I have a Consult with Dr. Juarez @ Phx. St. Luke. I have to go there due to the whole "Center of Excellence" required by Medicare. Hopes this helps.
so dr juarez takes phoenix health plan then? i have had such a hard time getting any answers and my dr bless her soul wants to help me badly, we have gotten statements from specialist in everything from gynos to bone dr's ect saying i need weight loss surgery but no one knows what dr takes phoenix health plan
I have my first Consult with Dr. Juarez Monday morning!!! So I haven't gotten the surgery approved yet, and had to write an Appeals letter just to get the referral. Phx Health Plan is also my secondary Ins. Everything has to go thru my Medicare plan (abrazo) first. The best advise I can give is get on the phone to your Insurance company and keep asking until you get an answer, Or call Dr. Juarez' office and find out from his staff which Ins he accepts. I have only talked to them twice, but they were very friendly and helpful. As you read thru these forums and others on other websites, you come away with tons of information. One thing is for sure, all the Insurance things are very different, so don't give up!
