Can anyone help? re: online medical information

I went through Bridges ( St Lukes) in Phoenix. When I was applying for this surgery I had to put all my medical stuff on line.
I need to know the web address so I can look up my medical information,
does anyone know what the address is?

I remember doing this two years ago, but I can't tell you the link.  Maybe call Kelly at Bridges.
