Help with Lapband approval. BCBSAZ

I was wondering if anyone has been approved for the Lapband procedure while insured by BCBSAZ PPO? The documentation is a bit confusing on their web site and I want to know how hard it is to get approved if someone went through it already. Thanks

Michael, I would definately call them and ask them what the exclusions are. A friend of mine has BC/BS of Arizona and unfortunately she is having an extremely hard time with them... but she is also looking into RNY, not lapband. So each case is different and depends on many factors. From my brief experience with BC/BS of AZ... the surgeon's office thought I had BC/BS of Arizona and told me I needed at least 6 months of doctor supervised weight loss records, strong documentation of weight lost attempts and of course a letter of medical necessity. I would definately get with your PCP and get some good documentation going. Each insurance company is so different now, its good to have all your ducks in a row. Good Luck to you! ~Nicole
