Bariactric doc or support group in Yuma?

Hi - I will be spending the winter in Yuma. I will have gastric bypass in Louisville, Kentucky, in October, and will need follow-up care and a support group when I get to Yuma. Does anyone know of a bariatric physician and/or a support group closer to Yuma than Phoenix? If not, who would you recommend in Phoenix for any follow-up care or problems? Thanks for your
