Anyone with Phoenix Health Plan get approved for Lap Band?

I called my insurance co. and they cover GB but they did say to go see my primary care physician and get a referral to see Dr. Simpson (the Lap Band doc.). Does that mean that there is a chance that they might cover LB surgery? Anyone ever deal with Phoenix Health Plan. Any advice or suggestions would be greatly appreciated. michelle
