Medicare & United HealthCare is Approval Possible? BMI 37

Wondering if anyone has been aproved with United HealthCare Choice Plus and or Medicare who has a BMI 36 to BMI 37. Also, would like to know the process of getting approved. I am just thinking about getting this surgery done and have not contacted anyone yet. From the website, I have seen profiles on two doctors in Arknasas who perform the surgery, I am sure that there are others, so any recomendation would be helpful. Also am wondering about out of pocket expense if UHC or Medicare does pay of the surgery, how much will it end up costing me anyway after insurance pays their part. Does anyone have an email address of a clinic that does obesity surgery? I am new to all of this and am in the learning process so I don't even know which type of surgery is best for me. I am 42 year old. I have learned a little about lap band surgery, but am not sure which type that is yet. Obviously want the surgery with the least complications as possible. Have no other conditions existing except dibeties in family and have had one high blood sugar in the past, but currently I think it's normal. Have tried diet pills and Nutri System but always regain what I loose plus some extra. Have read that others have been approved without extra conditons besides being overweight, so I think I could get approved by at least United HealthCare, not sure about Medicare approval. I am disabled with ADD, so I have medicare. Lots of questions, but in time, I am sure I will learn more and find answers to them all. Any and all suggestions and help would be greatly appreciated.
