Oh no! I am moveing to another city

I found out that we are moveing to another city. Now what should I do about the Arkansas Rehab.? Should I tell them that I am moveing to another city or should I keep an address and just stay with it here? I am so afraid this is going to mess things all up. What would you do?

Do any of you live near Ozark? It is about 40ish miles from Fort Smith?
