Letters from doctors?

Do any of you know where I can find some sample letters? I want to write the letters for my doctors and have them sign them. Just to make sure Ark. Rehab has all the information about me. I want them to know EVERY LITTLE and EVERY LARGE problem I have. I want them to know everything. You know? Paint them a very realistic, albeit graphic picture. So any help in wording, or what might be helpful for them to know. Things I may not even have thought of? I do not intend to lie at all. I just want them to know everything. To let them know what it is like to be in my shoes. If you know what I mean. Thanks.

Bonnie, Just a few things that might help. Get letters from your Dr's, especially from any Orthopedic Docs re' stress on your knees, back, ankles. They also want to see anything re any hypertension (high blood pressure). It would be better if you can get your Dr's to write your letters and them submit theirs with yours, and you just elaborate on the pain of walking, standing for long periods of times etc. Just tell the truth of the hell of an obese person goes through.
