Hi! Does anyone here have any suggestions on where to find resources for an appeal letter? HELP!!!!

Are you wanting to send an appeal from you to insurance, or from your doctor? If you want to send one yourself, you just compose a letter stating the facts , background, attempts at weight loss in the past, any health related problems and why you feel you should have this surgery. You need to be very open and detailed, dont hold back, but remember even though the first time it may not be successful you have to keep sending in appeals until they do it, unless there is a specific written exclusion. You can also have your pcp or surgeon or both send in letters. You can search the net and find sample letters that others have written to their insurance companies.Hope this helps.
