Need a letter

I am fighting with the review company that my insurance outsources to, InterCorp. WLS IS a benefit with my Insurance, was pre authed for it...then it went to IntraCorp...have done the psych eval, all labs, gallbladder scan, and cardiac clearance. IntraCorp now says the MD notes I submitted do not show monthly MD weigh in for 6 months! I have 2 months with no weigh in. I am seeing primary MD tomorrow and will ask him for documentation as to weigh ins. IntraCorp said that is ALL I need per review board. I have a surgey date of Aug. 12 and Ginger @ Dr Galvin office is holding it open. I need a letter to send with MD notes, was told there was one at this site. Any help?

I got a really good appeal letter from this site. I used it and got approved, so I thought it was wonderful!!
