Medicare requirements

Does anyone out there know what Medicare needs to get approval. I have been told you need to be under doctor supervised diet for 2 years first. Is there any other way I can get approval faster? I have already had both knees replaced and am destroying them already because of my weight, and now my hip and ankles are bothering me. I have high bloodpressure, depression, gurds, and severe back pain. Any info would help.
