Forgot Which Lab Sheet To Print Out (Dr. Baker's Patient Hub)

I got a letter in Dec for which lab protocol page to print out to take to my Dr for my 6 month labs, and I lost it.  Anyone know which one it is?  Is it lab protocol B or C?  Or is it called something else?  I opened both pages and it didn't seem like it was very much listed.  And I thought it was a longer list of labs. I am going to my Dr today so if anyone remembers which page to print out from Baker's Patient Hub site please let me know ASAP. Thanks

Nevermind.  LOL It's protocol B I have to print out. I feel dumb, hahahahahaha 

Hey what day do you go in for your 6 month check-up?  I don't go in until mid-March and I just got a letter reminding me of my appointment. 

I go Friday Feb 22nd.  I don't remember what time though, I got to call Monday and ask, LOL.
