It is called "Your Guide To Eating After Gastric Bypass Surgery" and it was made by Baptist Health.
This is page 7, Gastric Bypass Soft Diet Sample Menu, to give you an idea of what you will be eating like for the first 4 weeks after the liquid diet.

Okay to be honest with you, I didn't break up my 2 tablespoons into a teaspoon or 2 of this and that, I just ate 2 tablespoons of one thing, like for breakfast I would eat 2 tablespoons of cottage cheese or yogurt, then for lunch I would eat 2 tablespoons of tuna with some fat free mayo mixed in and put through the blender to make it smooth. And then for dinner I would have 2 tablespoons of macaroni and cheese chopped up real well, or pretty much whatever I could puree or chop into a smooth texture. I puree'd everything for a long time, even now I sometimes still put my meat through my magic bullet mixer, it just make it easier for me to eat.
