Curtis your records is over at RMC hospital. You can get your new doctor to send a request over or if your in this area stop in and sign a release. Make sure you tell them that you want your whole chart not just specific pages. Stress the whole chart.
Do you know how I can get my medical records from Dr. Freeman? I need to find another doctor to do my injections. Dr. Freeman did my lap-band procedure and I live in Columbus,Ga. Thank you!
Hello guys, I am new to this forum. I have a few questions.
My insurance, BCBSAL, says that I must undergo the 6 months of diets worded as follows:
Documentation of participation in a physician supervised program of nutrition
and increased physical activity (including dietitian consultation, low calorie
diet, increased physical activity and behavioral modification). Documentation
of program participation must appear in the medical record no less than
monthly for a period of 6 consecutive months by the attending physician.
Documentation should include comments by the physician regarding patient
progress or lack of progress. A letter does not meet this requirement. There
must be medical records to document medically supervised weight loss
attempts;
So in small terms, does that mean that I can visit my family physician once a month for 6 months for him to chart my progress and all that, or do I have to see a dietitian as well? Does my family physician also suffice in place of things such as weigh****chers or jenny craig, etc?
Any advice from anyone else with BCBSAL with getting certified to have surgery? I am 6'0 and weigh 347lbs. I have High Cholesterol, Hypertension, and Pre Diabetes. Hopefully it won't be too hard to get approval.
I, too, would like support groups that meet in Huntsville more frequently? Who did your surgery? I had mine done at Crestwood by Dr. Facundus, who is no longer there. I wonder if you can attend the Huntsville support groups even if your surgery was done at Crestwood. I know that Dr. Foreman has the practice Dr. Facundus had and that that support group meets once a month also. Let me know if you know of any others or if any of you would like to message with me. I have regained 50 pounds since my RNY in November, 2009.

I had RNY in o6 and in the past 2 years I have gained 45 pounds. I am trying very hard to lose these pounds but I lose some and gain them back. Help, what can I do? I do have health issues that prevent me from getting enough exercise and I have hypothyroid but I take meds for that. Does anyone know if it is safe to take Garcinia Cambogia after gastric bypass? Has anyone tried it? Thanks

My name is Michelle and I am from Tuscaloosa. I have been a member of OH since 2004, but I have long since stopped being an active member. Looking back over my posts, it is amusing just how much different I am from 10 years ego (at a mature 19 years old). I first began trying to have weight loss surgery in 2003 at the age of 18, and joined OH in the following year. The process had begun with Dr. Bradley Bilton and Beverly Childers (now Beverly Office) and I lost my insurance and sort of dropped all of the progress and, really, the hope of having surgery. Fast forward 10 years, and I work at the University of Alabama (from where I also got a bachelor degree....and in one of my previous posts, I was using my weight as an excuse not to go college...hmm.), am married, and have my own insurance (very good insurance, in fact).
Two days ago, I got the confirmation from Beverly that my surgery has been approved. I am in shock. This was the point with my previous attempt where I was shut down and gave up. Now I have been approved and I have pre-op class next Wednesday. This is a day I never thought I would see.
Anyone else still around from 2004 (ish)? I hope everyone remains well and I am beyond glad to be back here, and finally, successfully getting a second chance to be healthy and happy!

Hang in there! Exercise is crucial, when i can't get to the gym or I'm pressed for time I do a burst workout, you can google it. It is very affordable and fun.
Hi! Sorry that it took me so long to respond! I watched his seminar. It's prerequisite to scheduling an appointment with him. I have not been to a support group yet though. My first appointment with him is April 10th. I know it's going to be at least 7 months after my first appointment before I can send off for insurance approval, but I'm very anxiously waiting. :)
A friend of mine is having Dr. Schmitt do her surgery on Wednesday and she has said nothing but wonderful things about him. She says he is very knowledgable. Dr. Miles (my surgeon) is also very knowledgable.
A small plus with Dr. Schmitt, he only makes you do 1 week pre op liquid diet and Dr. Miles makes you do 2.
