I am just now exploring the surgery, but I do not know if I qualify.
I know that you must show a three year record of obesity. Almost two years ago, I starting working out like crazy and lost 50 pounds, sending me to 275 and below the required BMI of 40. I broke my back and shot back up in weight. When my doctor shows the records of obesity, does he have to show them every weigh in, or just an instance of obesity levels over three years (one per year, two per year, all?)?

You may want to call your insurance and get some clarification. For my insurance I had to show records for over two years of being over 35 BMI and 6 months of Dr supervised weight loss and the numbers.
Now some require a BMI of 35 with two comorbilities or a BMI over 40 with no comorbilities

I also have the same insurance you have. I had to go back 3 years for medical history, and do six months of a supervised diet/weight plan. I qualified with the pre-existing morbid conditions.I had no problems with paperwork them and was approved almost immediately! Good luck, and I'm sorry about your back issues. I have arithritis in my back, so I feel your pain,
