I had approval before surgery!  I do not understand this!  I certainly can't afford the $40,000+ bill.  Has anyone else dealt with this?

Appeal!!
I had a denied hospital claim for my revision. I appealed and won the appeal for them to cover hospital cost but they would not cover physician costs.
Remember you only have so many days to appeal this decision so you need to get right on it. Document, certified mail, ect. Keep all your records!! I had a hard road but I assure you a $30,000 bill in my case was worth fighting for it. I lost on the physician part, due to the physician not filing their claim within the 90 day window. (or so my insurance claimed)
I hope this helps you with your case. Please document and send all mail certified with receipts for you to keep!!

mschelle. I do hate to tell you this but just because you got a pre-cert # does not in any way guarantee payment.  If you will go back and reread your pre-cert they will probaly tell you that. How do I know?  My daughter in law went thru the very same thing and the insurance refused to pay one penny. She had a pre-cert #, letter, and everthing else that she needed. So yes it does happen. She is 3 years out is still paying the hospital ever month. The doctor wrote his fees off.  I would call the hospital and ask and see if it was somethig due to coding. Also call the insurance company and talk with someone and see just what they need. Could be just a mix up.
