Address List - Added Information

I am trying to get the database set up now. The report may be a little lengthier than the last one, but I hope that I will be able to run seperate reports, like just a birthday list for example. I know I'm not on here much, but I agree with Carmen that there are some folks on the current list that I have never heard of. I will do my best to help and keep it current.
