Family History/Psych eval

Hey folks, I need some opinions. In a couple of weeks, I am going to have a psych eval done, even though it is not required by my insurance. I just want to do it so it will give the insurance one less reason to deny me.
This is my question/dilemma: On the family history section, it asks about any history of drug/alcohol abuse or dependency. While I don't have any issues - have never done drugs and I may have one drink a year - my sibling did have issues with drugs. Fortunately they are both clean now, but they did have drug habits for several years. Also in my family history is a grandfather who was an alcoholic.
So, my question is, should I mention these things, since they are asking about them? As I said, I don't have any of these issues and I don't want them to recommend postponing surgery dependent on having counselling.
I know about the chances of post-op transfer addictions, but since I haven't had any addiction issues thus far, i don't necessarily think I am at a higher risk than anyone else.
What would you do?

You should never hold anything back when you are talking to a psychiatrist/psychologist. They cannot help you or realistically evaluate you if you are forthcoming with the whole truth. Otherwise, why bother with the consultation? Your family's history just means you are at risk. It doesn't mean you will definitely have problems yourself so just be honest. If you do run into problems down the road with transfer addictions, then you can deal with it at that time.

Thanks, Sandy - I do see your point. I have decided to just lay it all out there and see how it goes from there.
