detailed receipt for health care from specialized bariatrics

i went through specialized bariatrics for my surgery and they are telling me that they only have a lump sum package amount and cannot give me a break down of charges and with out a break down of charges i cannot claim it with alberta health or taxes. anyone ever get a broken down receipt from them and who did you speak with/who was your coordinator. i am pretty sure all my claims will be rejected with out this info and i am sure that i once saw someone say they had gotten a break down with them but i cannot find it.

I am going through them as well for my surgery. I found that the price break down was in the full package I downloaded and printed off that they sent me. My coordinator is Cathy. 

I also went with Specialized Bariatric and am having the same problem if you would also be willing to send me a copy I would really appreciate it.

Cathy is my coordinator to what do you mean the break down is in the full package. Did they break it down suffieciently to use with alberta health because my invoice hase a lump sum and no break down. Any chance you could private message me a copy of what you got (personal info covered of course) or your name so I could ask for a break down like yours

I just got off the phone with Alberta Health and they were so friendly and polite.  She also explained the reimbursement would depend - am I in a hospital or in a clinic?  She told me that I should ask for a specific breakdown off all lab tests, procedures, with names and times.

Does anyone have any specific tips?  My surgery is Sept. 2nd.  Very excited :)

I'm trying to decide if I should get this done by Dr.Joya in Puerto Vallata. I finally saved enough I hope. I would like to have a date picked soon. How did you decide to have Dr.Joya do the procedure. How do you know he is legit too.

Ps how did you pay the down. I mean did you just give them your card number ...

based on my expirence with specialty bariatrics i would request a receipt in advance with the break down and not use specialty bariatrics they will not give me a break down

I just sent an email to Cathy asking for a detailed receipt and the process around that. The front page of my package has a list of what is all included in the cost of the surgery. Besides the issue with the receipt how was your experience with your surgery? Or have you not gone down yet? I am due to go for October 15th. Once I have a reply from Cathy about the detailed receipt I will follow up with you.

the front page does not help with a reembersment or taxes because it does not break down the amount per service only a lump sum amount.
