Just had my seminar
I had my seminar today (so GLAD). There is a form I have to fill out. On this form they want to know what types of programs that you have been on in the past to lose weight or pills you have taken for this. What happens if it is too few (I have been to Weigh****chers 3 times, Diet workshop, and took pills obviously with no luck). Will he not take me because I haven't tried "X" amount of things? I am really worried about this (yeah I know kind of dumb but this is haunting me). Thanks for your opinions and advice!

Congratulations on starting your journey. I hope all goes smoothly for you.
As to the form.... find out what your insurance requires for diet documentation first. Knowing exactly what they are looking for will help you write down things based on their requirements. If you haven't done so already, call the number on the back of your insurance card and simply ask them if WLS is covered and what you need to do to be approved. Then ask them to send that information to you in writing. This will help you get through the approval process easier --- you can use that letter as a checklist of sorts.
The forms they gave you will help you fulfill the insurance requirements. It will also show the surgeon that you've made attempts in the past to lose weight. When I was preparing for surgery I made a list for my own personal peace of mind. I included as many different diets that I could think of that I'd ever tried (low carb, low fat, cabbage soup, food pyramid, etc.) and how long I was on each and how much I lost. It took me several days to rack my brain and remember everything for as far back as possible ... but having all that down in writing really helped me solidify in my mind that I'd done everything possible. This list will help you also figure out how to complete those forms for the surgeon's office.
